A person with an opioid use disorder needs medical attention and goes to a hospital, out-patient facility or clinic. They receive the needed intervention to stabilize their immediate condition.

Stage 1: Identification

Stage 2: Screening / Assessment / Level of Care Determination
Needs Assessment
A person with opioid use disorder is evaluated and their needs are assessed. An OUD treatment plan is formulated and appropriate community support services are determined.

All within the EHR, Pieces Predict™ flags patients for being at risk for OUD which allows for prioritization for opioid Screen, Brief Intervention, Referral & Treatment (SBIRT)* processes. Results of opioid screening informs the recommended opioid treatment plan.

Results from the SBIRT are shared as part of referrals to the Opioid Care Network providers for increased communication, transparency, and coordination. Detailed reports are provided for all stakeholders.
Stage 3: Treatment
Coordinated Care
A person with opioid use disorder gets referred to the appropriate community resources. With their consent, their opioid treatment plan is shared with the needed support organizations so treatment can be coordinated across multiple care providers.

Pieces Connect™ tracks the patient through their particular Opioid Care Network for coordinated care across multiple treatment services and recovery support organizations. Loops are closed on referrals automatically when the person arrives for appointments. This eliminates any need for follow-ups by the case manager.

Stage 4: Management
Case Management
A person with OUD utilizes the needed medical, social, and emotional support to address their needs. Their information goes with them electronically from provider to provider. As they move toward recovery, the opioid recovery service options are presented for ongoing support.
